Xamic 500 is indicated for the management of bleeding in various conditions, including:

• Heavy menstrual bleeding in women
• Bleeding inside the digestive system (e.g., inflammatory bowel disease)

• Hematuria (blood in urine) not caused by kidney parenchyma
• Bleeding control after dental extractions in patients with coagulopathies

✔️ Pharmacology

Tranexamic acid (trans-4 aminomethyl-cyclohexanecarboxylic acid) is a strong antifibrinolytic agent, effective in inhibiting the activation of plasminogen to plasmin, which is essential for clot breakdown. It acts by preventing the binding of plasminogen to fibrin, thereby inhibiting fibrinolysis. Tranexamic acid is more effective and long-lasting than conventional hemostatic agents due to its slower excretion via the renal route and higher potency. It does not interfere with normal clotting processes and has low acute and chronic toxicity.

✔️ Dosage & Administration of Xamic 500

Adults:

• Usual dose: 500-1000 mg three times daily.
• Prophylaxis: 0.5-1 g daily orally, or 500 mg intravenously or intramuscularly.
• Therapy for hemorrhagic manifestations: 1-3 g daily in divided doses. In severe cases, start with 500 mg intravenously, followed by oral doses as required.

Children:

• Prophylaxis: 5-10 mg/kg daily in divided doses.
• Therapeutic: 10-20 mg/kg daily orally, or 10 mg/kg intravenously or intramuscularly. Oral administration of diluted ampoules is an option for small children.

Elderly:

• No dosage reduction is necessary unless there is renal impairment.

• Antagonistic drugs: Thrombolytics like streptokinase and urokinase.

• Use with caution in patients with hyperfibrinolysis.
• Prophylactic treatment should start 24 hours before surgery and continue for 3-4 days post-operation.
• For hemorrhages, continue therapy for at least 24 hours after symptoms subside.
• Reduce doses in hematuria to prevent urinary tract clots.
• Avoid use in severe renal dysfunction; use cautiously in mild to moderate renal impairment.
• Requires careful administration in patients with heart or liver conditions.
